ICHP/FCCPT/NBCOT Visa Screen

FCCPT Interpretation Statement on Master's Degree Equivalency (if acquired)

FCCPT credentialing report include VisaScreen again if Type I (If acquired)

OTED credentialing report from NBCOT (if acquired)

NBCOT certificate (if acquired)
All English score reports (if applicable)

US License or eligibility Letter (if acquired)

For therapy, NPTE or NBCOT Passing Scores (if acquired)

ATT (if candidate is eligible to take NPTE or NBCOT, if acquired)

If filed for H1B previously;

USCIS Receipt notices

USCIS Approval Notices

Copy of I94 card (stapled on Passport)

If H1B clarify if they were filed through regular H1B cap or were they
filed through an H1B-Cap Exempt Employer.
