Certified Nurse Practitioner/Physician Assistant

We are seeking a hardworking, dedicated, reliable NP or PA to join our practice. We are a private home healthcare provider service that work in a collaborative spirit to provide primary prevention, secondary prevention, and treatment of chronic issues. We primarily see homebound adult or disabled patients. We are an acute intervention medicine unit that specializes in diagnostic testing. We work closely with home health agencies, physicians and neighborhood organizations that reach out to the community.

We are located in the chicagoland area. A span of 30-100 miles may be covered within one day of visits to homes. The mid-level provider will be traveling with a team, and will function as the team leader. Physicians are available for collaborative agreements and for consulting about the care of the patients. Our patients are appreciative and welcome the opportunity for a wellness visit. Nurse Practitioner or Physician Assistant must have a valid Medical State of Illinois License.
