Per Diem Social Worker for Hospice The selected candidate will work as part of the interdisciplinary hospice team. This position covers Springfield, MA and surrounding areas. Requirements: Minimum one year of medical social work experience in healthcare setting. Hospice/long term care experience, a plus. MSW and LICSW required. PC literate and excellent documentation skills. Strong communication skills. Valid driver's license and ability to travel locally within assigned territory. Knowledge of Medicare/Medicaid, helpful.
