Where do you get these parts from? Is this an Apple LCD? 

It's an original, not a knockoff! Sharp LCD + quality digitizer.

Many knockoffs exist. Some don't use the original Sharp LCD. Others use a digitizer with a visible digitizer "grid" - there is a grid on the glass, that is used to tell where you are touching the phone. A visible grid means it is a low quality digitizer that cannot sense precise touches as well. We use the original Sharp LCD and a high quality digitizer with no visible grid, just like that which was on your iPhone from the factory!

We also have no proximity sensor issues. The proximity sensor turns your phone's screen off when it is near your face, so that your cheek or chin don't hit end call, mute, and speaker when you are on the phone. With other vendors, putting the phone to your ear post repair will find you in hell. The phone's screen will not go off. Our parts are not knockoffs, and do not cause such defects.

What kind of warranty does this come with?

If the LCD itself malfunctions within 1 year, you'll get a new one installed for free.
