Schedule: Monday - Friday 8:30am-5pm
NATURE OF WORK:
Primary Function: Responsible for the provision of Home–Health Care casework services to qualified individuals. This position is responsible for the provision and/or arrangement of an array of Home-Health Care services.
KNOWLEDGE, SKILLS AND ABILITIES:
Good knowledge of the practices and procedures of social casework and of social factors affecting individuals and families; good knowledge of interviewing and counseling techniques; good knowledge of the methods used in the development of case plans; working knowledge of the Federal, State and Local laws affecting the social casework program; working knowledge of social, health, vocational and other related agencies and programs in the area available for assistance and referral; ability to establish and maintain effective working relationships with a wide variety of persons, including clients, social workers, medical and legal professionals, government and law enforcement officials, and members of the general public; ability to assess clients’ needs and develop sound and effective Home Health Care plans to meet those needs; ability to deal with a wide variety of people with an extraordinary amount of tact and courtesy; ability to keep case records and prepare reports; ability to organized and plan work effectively; ready access to transportation to meet field work requirements; good powers of observation and analysis; initiative and resourcefulness; creativity and imagination; emotional maturity; good judgment.
- Bachelor’s degree in Psychology, Social Work Health Care or a related Human Service field, with preference given to direct experience working within the Home Health Care model of Services.
- A valid driver’s license passing insurability guidelines.
