Maintain accurate, detailed reports and records of patients at skilled nursing facility. Will discuss treatment and prescribe medications with patients, family and other health care workers. Administer medications to patients and observe patients' reactions or side effects. Records patients' medical information, lab/test results and vital signs. Monitor, record and report symptoms or changes in patients' condition. Consult and coordinate with healthcare workers to assess, plan, implement or evaluate patient care plans. Modify patients' treatment plans as indicated by patients' responses and conditions. Observe all aspects of patient care, including diet and physical activity. Prepare patients for and assist with examination or treatment. Instruct individuals, families or groups or topics such health educations, disease prevention and develop health improvement programs.
