Currently searching for RN or SW Case Manager for completion of assessments, incident reports, and home visits. Currently searching for RN or SW Case Manager to serve the Wake County area. The candidate must have at least full time experience of direct clinical care preferably in home health and pediatrics.  Driving is a requirement of this position.  This can be a partial work from home opportunity.  Hours are flexible with occasional later afternoon / early evening appointments. The RN/SW will conduct assessments with medically fragile and chronically ill children birth to 21 years of age.  The Case Manager is responsible for providing case management services which facilitate the delivery of quality services.  The Case Manager collaborates with the client, family, medical providers, and other resources so the child can remain at home through advocacy, assessment, planning, communication, education, resource management, and service coordination / implementation.  Must have the following:  A current NC Registered Nurse (RN) license.  A Bachelor's degree in social work, a Bachelor's degree in a human services field, or equivalent field from an accredited college or university.  A broad range of knowledge of medical diagnoses/impairments is desirable; prefer case managment, pediatrics, and/or home health experience.  Excellent computer skills are required.  Contract full or part-time position.  Please respond with resume to contact@amazingkidsofnc.org or fax (919) 263-8503.
