Requirements:

  • Hold a Bachelor's degree or higher in Nursing, demonstrating your dedication to your profession.
  • Possess a valid Registered Nurse (RN) license/certification in the state of New York (NY), ensuring your compliance with regulatory standards.
  • Bring a minimum of 2+ years of nursing experience in a related field, showcasing your expertise.
  • Exhibit specialized experience of 2+ years in either Medical-Surgical (Med-Surg) or Home Health (Home Care), highlighting your specific skills.
  • Own a car, a NY-issued driver's license, and appropriate auto insurance, enabling you to provide seamless care.

Responsibilities:

  • Deliver direct patient care aligned with policies, goals, objectives and the Standards of Nursing Practice.
  • Employ your expertise to administer wound care, manage IVs and catheters, and administer medications effectively.
  • Embark on 5-7 home visits per day, ensuring thorough and attentive care.
  • Conduct comprehensive assessments of patients' physical, psychological, and social needs, translating them into effective care plans.
  • Collaborate harmoniously with the interdisciplinary team to ensure holistic patient well-being.
  • Manage the scheduling and execution of patient visits, including start-of-care and revisits.
  • Thoroughly document all provided services, ensuring compliance with governmental regulations and Health policies and procedures.
  • Participate actively in regional team activities, including providing orientation support and receiving performance feedback.
  • Foster effective communication with team members, management, and regional leadership.
  • Travel to patients' homes to deliver care, adhering diligently to established schedules and protocols.
  • Uphold the principles of patient advocacy, consistently safeguarding patient rights and dignity.

The client offers an extensive 2-4 week orientation to ensure you are fully equipped for success in this role.
